Crash Report
Crash Detail
The Crash Detail section is used to enter exact location, crash/notified/arrived date/time, officer, and answer specific questions about the crash being reported.
Vehicles
The Vehicle section is used to enter any vehicles that were involved in the crash being reported. A Search for a vehicle by tag number or VIN can be performed to return details about the vehicle as well as the vehicle status (e.g. stolen vehicle). Check 'Use Vehicle In Report' to import the vehicle information into a new vehicle. Click on the 'TIES Response' link to view the entire result that was returned.
When multiple vehicles exist, the numbering order of vehicles can be changed by using the arrow buttons (located next to the 'VIN' field). When a vehicle number is changed, the numbering order for all other vehicles is automatically updated.

Driver
The Driver section is used to enter information about the driver of the vehicle that was involved in the crash being reported. A Search can be performed by either SS# or both Drivers License # and State. The search will attempt to find the person in the local and NCIC databases and, will retrieve possible warrants. If a record is returned, check the 'Use Info' box to use the results to populate the form. If 'NCIC results didn't return yet, try again' appears in the lower half of the search results, try performing the search again. The NCIC results may have not been returned in a timely manner and therefore not immediately available. Click on the 'TIES Response' link located near the center of the screen to view the entire result that was returned. The user may also create an Arrest Ticket by checking 'Generate arrest ticket for this person' if warrants are found. This will create an independent arrest ticket outside of the crash report. If the person was not found, enter the data into the appropriate fields. A contact phone number is required (e.g. Home phone #, cell phone # or work phone #). If no contact number is available, enter 'None' or 'Unknown'.
Passengers
The passenger section is used to enter information about any other person(s) that were in the vehicle. Refer to the Driver section for data entry instructions.
Owner
The Owner section is used to enter information about the owner of the vehicle. Refer to the Driver section for data entry instructions. If the Driver or Passenger is the owner of the vehicle, check the 'Person is Vehicle Owner' check box in the Driver or Passenger sections.
Non-Motorists
The Non-Motorist section is used to enter information about any pedestrians, cyclists, or other non-motorist types involved in the crash. Refer to the Driver section for data entry instructions.
Witnesses
The Witness section is used to enter information about any persons who may have seen, heard, or have any information about the crash. Refer to the Driver section for data entry instructions.
Other Damaged Property
The Other Damaged Property section is used to enter any property that may have been damaged (e.g. fence, sign, pole).
Personal Statements
The Personal Statement section is used to either enter or record a personal statement given by persons involved in the crash or the reporting officer. Select the 'Record' button to capture a recording for the statement that is being given. The maximum length of the recording is 15 seconds. After the recording has been captured, The 'Re-record', 'Play', and 'Del' buttons appear allowing the user to re-record, play back, or delete the recording that was captured. All recordings must be played back before they can be saved and validated. Storage and submission of recorded personal statements is handled by the application.
Diagram
The Diagram section is used to enter a drawing of the crash scene. The diagram can be submitted in one of the following ways:
Narrative/Case Notes
The Narrative/Case Note section is used to enter a detail description about the crash. Case notes should be entered in addition to a short narrative when reporting items that are sensitive in nature. The purpose of case notes is to prevent the release of information that may compromise the investigation. The public and media have access to all Field Offense Report narratives. DO NOT enter the 'Case Notes' in the narrative section. Keywords such as names, vehicles, properties, etc from the crash can be selected from the 'Keywords' drop down list for use in the narrative.
Photos
Add photos of the crash scene utilizing the built in camera of the handheld device (if available). Upon opening the photo section, select 'New' to launch the camera and take photos. Once the photo(s) have been taken, and the camera application is closed, they can be viewed in the photo window. Photos can then be saved and validated or deleted and the photo retaken if necessary. A maximum of 4 photos can be taken per item per report/continuation. Any photos that have been taken since the report was started/re-opened will show in the Photo wizard.
Continuation/Amendment
Continuations can be used to add items to a Crash report that has been submitted and approved by a supervisor. Amendments are used to add or edit items in a report that has been submitted. Once the report has been amended, it must be re-submitted.
