Incident/Memo Report
Incident/Memo Detail
The Incident/Memo Detail section is used to enter the location, date/time, officer, and answer specific questions about the incident being reported.
Incident reports can be related to a crash report by selecting the 'Relate to Crash' option. Upon choosing the crash that the incident will be related, data from that crash report will be available for re-use within the new incident report.
The 'Copy from Incident' option allows the user to copy the basic information from a previously submitted incident report into a new incident report. The user can either download the report that is to be copied or use one that is already stored on the machine.
A Memo is used for reporting miscellaneous incidents that may need documenting for various purposes. Memos include all the same items as a regular incident report except for offenses (Offenses and Arrests cannot be added to a memo).
Offenses (Incident only)
The Offense section is used to enter any offenses that occurred in relation to the incident, whether they were completed or attempted, and answer specific questions about the offense. The application will rank offenses in hierarchal position as you enter them. When you pick the offense category the offense list will populate with the relevant offenses. Each TIBRS code can only be entered once. In some offenses such as, 'Counterfeiting/Forgery' or 'Drugs/Narcotics', Society/Public will automatically be added to the Victims list. It cannot be altered or removed without removing the offense. In the offense of 'Kidnapping/Abduction', Personal Freedom will automatically be added to the Properties list. It cannot be altered or removed without removing the offense. In the offense of 'Arson', a property for arson will automatically be added to the Properties list.
Complainant
The Complainant section is used to enter information about the complainant(s) who are involved in the incident being reported. A Search can be performed by either SS# or both Drivers License # and State. The search will attempt to find the person in local and NCIC databases and will retrieve possible warrants. If a record is returned, check the 'Use Info' box to use the results to populate the form. If 'NCIC results didn't return yet, try again' appears in the lower half of the search results, try performing the search again. The NCIC results may have not been returned in a timely manner and therefore not available. Click on the 'TIES Response' link located near the center of the screen to view the entire text result that was returned. The user may also create an Arrest Ticket by checking 'Generate arrest ticket for this person' if warrants are found. If the person was not found, enter the data into the appropriate fields. If the 'Copy From Incident' or 'Relate To Crash' functions have been used, the SSN field has a dropdown that contains a list of people from the report that was copied/related. A contact phone number is required (e.g. Home phone #, cell phone #, work phone #). If no contact number is available, enter 'None' or 'Unknown'.
Victim
The Victim section is used to enter any person or business that was victimized in the incident being reported. For data entry instructions, refer to the Complainant section
Suspect
The Suspect section is used to enter offenders that were involved in the incident being reported. For data entry instructions, refer to the Complainant section.
Witness
The Witness section is used to enter any persons who may have seen, heard, or have any information about the incident that is being reported. For data entry instructions, refer to the Complainant section.
Property
The Property section is used to enter details about any items that may have been stolen, recovered, damaged, or destroyed during the course of the incident being reported.
Drugs
The Drug section is used to enter any drugs or drug paraphernalia that may have been recovered. A drug offense must be added before a drug can be entered in to the drug section
Vehicle
The Vehicle section is used to enter any vehicles that were involved in the incident. Some offenses do not allow vehicles to be related to them. A vehicle can be added for investigative purposes only (not related to an offense).
A Search for a vehicle by tag number or VIN can be performed to return details about the vehicle as well as the vehicle status (e.g. stolen vehicle). Check 'Use Vehicle In Report' to import the vehicle information into a new vehicle. Click on the 'TIES Response' link to view the entire result that was returned.
If the 'Copy From Incident' or 'Relate to Crash' functions have been used, the VIN field has a dropdown containing a list of vehicles from the report that was copied/related.
Worthless Documents
The Worthless Document section is used to enter items such as counterfeit money, credit cards, and forged checks that were used in the incident being reported. This section can only be used for Counterfeiting/Forgery, Prescription Forgery, and Credit Card/ATM Fraud offenses. One of the previously mentioned offenses must be added before a worthless document can be added to the report.
Narrative/Case Notes
The Narrative/Case Note section is used to enter a detailed description about the incident being reported. Case notes should be entered in addition to a narrative when reporting crimes that are sensitive in nature, such as homicides, child abuse and sex offenses. The purpose of case notes is to prevent the release of information that may compromise an ongoing investigation. The public and media have access to all Field Offense Report narratives. DO NOT enter the 'Case Notes' in the narrative section.
Keywords such as names, offenses, properties, etc from the incident can be selected from the 'Keywords' drop down list for use in the narrative.
Arrest Ticket (incident only)
The Arrest Ticket section is used to add an arrest for any persons being arrested. A Search can be performed by either SS# or both Drivers License # and State to get arrestee information (see Complainant for search details). An Arrest Ticket can be created within in Incident (related to and Incident) or as an Independent arrest (based upon warrants alone) (see Start New Arrest). In order for and arrest to be added to a report, a suspect must have already been added. If supervisor approval is required for the arrest, the supervisor must enter the login id and password that has been assigned by an administrator, and then select 'Approve'. Some jurisdictions will have the 'Misdemeanor Citation' printing function available within the Arrest ticket. Select the Court and the Date/Time of the court appearance. Select 'Print' and the citation will be printed (printer must be available). When an arrest is related to a Crash report, any charges that were added in the report will be imported into the arrest ticket.
Photo
Add photos of the incident scene utilizing the built in camera of the handheld device (if available). Upon opening the photo section, select 'New' to launch the camera and take photos. Once the photo(s) have been taken, and the camera application is closed, they can be viewed in the photo window. Photos can then be saved and validated or deleted and the photo retaken if necessary. A maximum of 4 photos can be taken per item per report/continuation. Any photos that have been taken since the report was started/re-opened will show in the Photo wizard.
Continuation
A Continuation can be used to add additional offenses, victims, suspects, witnesses, properties, worthless documents, narratives, and photos to an original report. Continuations can only be added to Incident Reports that have been submitted. The Incident number (case number) will remain the same for the continuation as it was for the original report.
