Enter the name of the entity responsible for the safe movement of the vehicle.

If the responsible party is a State or local government agency or school system, enter entity name in the "Carrier Name" field. Enter street address, city name, state and zip code of the entity.

If the responsible party is a private individual, enter "Individual" in the "Carrier Name" field. Enter "Unknown" in the "Street Address" field.
